Facilities Involved in the ICH CAHPS QIA

Project Guidelines

Project Agreements/Contracts due back to the Network by March 17, 2017

In order to comply with federal regulations issued by the Centers for Medicare & Medicaid Services (CMS), chronic out-patient dialysis facilities that treat more than 30 eligible patients during the prior calendar year are required to participate in the In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) survey (Agency for Healthcare Research and Quality, 2015). The 62-question survey is designed to measure the experiences of people receiving in-center dialysis. Questions focus on four key components: Kidney Doctors, Dialysis Facility Staff, The Dialysis Facility and Dialysis Treatment. All ICH CAHPS activities are carried out by third party vendors that administer the survey twice a year (spring and fall).

The goal of this project is to improve communication between patients and staff so that patients become more involved in their care. The Network plans to work with providers to engage patients in creating goals to be included in their plan of care. It is CME's belief that working with patients to create goals will improve their experience of care, ultimately positively affecting questions 10-18 of the ICH CAHPS survey. The Network will use data available from the Spring 2016 ICH-CAHPS to select facilities that have the lowest aggregate scores on these questions in the Quality of Dialysis Center Care and Operations section of the survey.

AIM:  The Network shall demonstrate improvement by achieving at least a 5% relative improvement in the number of patients who 1) set a goal and 2) achieve their goal

Baseline: Spring 2016 date provided by NCC with re-measure in September 2017

Goal and Timeline: The network shall demonstrate improvement by achieving at least a 5% relative improvement in the number of patients who 1) Set a goal and 2) Achieve their goal
