Independent Facility Status Reporting: COVID-19

Understanding COVID-19 prevalence and trends in the ESRD community is critical in helping to build a comprehensive approach for supporting dialysis treatment, staffing, and access to care for patients during this public health emergency.

INDEPENDENT FACILITIES ONLY: Centers for Medicare & Medicaid Services (CMS) requires ESRD Networks to track and report impact on independent dialysis facilities. Please complete the COVID-19 Facility Status Report EACH DAY by close of business by going to Network 1 Independent Dialysis Facility Status Report: COVID-19.

NOTE: Large Dialysis Organization (LDO) facilities - i.e. Fresenius Kidney Care, DaVita, DCI, DSI, American Renal Associates, and US Renal Care - are required to report impact directly to their corporate emergency coordinators which is being shared nationally. Any impacted facility will be TEMPORARILY removed from QIA projects to focus efforts on managing COVID-19 cases and/or preventing the spread of this infection to facility staff and patients. Network staff will follow up with facilities to help address emergent needs and/or to provide appropriate resources.

Please send any questions regarding COVID-19 impact to esrdnetwork1@ipro.us or call 203-389-9332.
